Abstract
The needs of families and individuals managing chronic diseases are complex and varied. Christian churches have opportunities through their understanding of health and wholeness to provide assistance to families and individuals with care giving needs. This has led to the development of an educational program for churches to assess, educate, evaluate, and implement a plan to help individuals and families with care giving needs. This tool helps Christian churches to put their faith in action as they consider their role in helping families with the management of chronic disease.
